OT 105 - Documentation in Occupational Therapy Description This course provides an introduction to the documentation process for occupational therapy assistants with an emphasis on developing skills for clinical practice to meet professional standards and reimbursement guidelines. Additional topics include clinical reasoning skills, role delineation in documentation, use of professional terminology, and ethical and legal considerations related to the medical record. Electronic medical records and their characteristics and use are also addressed. - Explain the role delineation and responsibilities of the OT and OTA for documentation in various practice settings.
- Apply commonly used abbreviations and OT terminology in documentation of services in the health record.
- Differentiate between an initial evaluation, progress note, contact note, and discharge summary.
- Demonstrate the ability to develop treatment objectives, goals, and interventions to meet standards of skilled care.
- Define and demonstrate writing the four components of the SOAP note (“S”-Subjective, “O”-Objective, “A”-Assessment, and “P”-Plan).
- Contrast various styles of documentation including SOAP and narrative formats .
- Define and demonstrate knowledge of electronic health record (EHR) or electronic medical record (EMR) (2018 ACOTE Standard B.4.15).
- Demonstrate knowledge of various reimbursement systems and funding mechanisms treatment/diagnosis codes, and coding and documentation requirements that affect consumers and the practice of occupational therapy (2018 ACOTE Standard B.4.29).
